ABSTRACT

Scholars around the world have dedicated themselves to developing an effective vaccine against SARS-CoV-2. However, vaccines have produced adverse effects in some patients. We report the case of a 44-year-old man who developed a pruritic papulosquamous rash on the trunk with a characteristic pattern known as Christmas tree after receiving the first dose of the Oxford-AstraZeneca COVID-19 vaccine, similar to pityriasis rosea (PR). He had no previous symptoms of viral infection and tested negative for neutralizing antibodies (enzyme immunoassay) against COVID-19. There are few reports in the literature about the relationship between the onset of cutaneous adverse reactions and the Oxford-AstraZeneca vaccine, therefore, the dissemination of this case is of paramount importance.

ABSTRACT

A completa remoção cirúrgica do lentigo maligno é desafiadora devido à dificuldade em determinar margens, disseminação subclínica e frequente localização facial. Neste artigo, relata-se abordagem cirúrgica de extenso lentigo maligno facial pela técnica de spaghetti, com margens livres no terceiro estágio cirúrgico. Essa técnica consiste na ressecção de margem circular periférica aos limites clínicos do tumor com avaliação histológica, sem remoção da lesão. Novas etapas incluem ampliação das margens comprometidas até que todo o perímetro esteja livre, quando então a ressecção do tumor e a reconstrução do defeito cirúrgico são realizadas simultaneamente. Objetiva atingir margens livres preservando tecido em áreas estéticas e funcionais.


Complete surgical removal of lentigo maligna is challenging due to the difficulty in determining margins, subclinical spread and frequent facial location. In this article, we report the surgical approach of an extensive facial lentigo maligna with the "spaghetti" technique, with free margins in the third surgical step. This technique consists in the resection of a circular margin surrounding the clinical boundaries of the tumor, with histological evaluation, without removing the lesion. New steps include widening of the affected margins until the whole area is free, when tumor resection and reconstruction of the surgical defect take place simultaneously. The aim is to reach the free margins, preserving tissue in aesthetic and functional areas.

ABSTRACT

A evolução nas técnicas cirúrgicas do tratamento do vitiligo tem proporcionado resultados extremamente satisfatórios em pacientes com vitiligo estável, sem fenômeno de Köebner e refratários a outros métodos terapêuticos. Diversas técnicas são conhecidas e devem ser adequadas de acordo com o paciente e a experiência do dermatologista para otimizar os resultados obtidos.


The evolution in surgical techniques for treatment of vitiligo has been producing extremely satisfying results in patients with stable vitiligo, without Köebner phenomenon and resistants to other types of treatment. Many techniques are available and should be used accordingly to the patient and to the dermatologist experience to optimize the results.

ABSTRACT

Em um estudo aberto, quatro pacientes foram selecionados para avaliação da eficácia do imiquimod creme 5% no tratamento do carcinoma basocelular superficial. Os pacientes utilizaram o medicamento no regime de uma aplicação diária, por 12 semanas. A taxa de resposta foi avaliada pela remissão clínica e histológica. O uso do imiquimod creme a 5% no tratamento do carcinoma basocelular resultou na remissão completa dos tumores neste estudo.


In an open label study 4 patients were selected in order to evaluate the efficacy of imiquimod cream 5% in the treatment of superficial basal cell carcinoma. The patients applied the medication once a day for 12 weeks. The response rate was evaluated through the clinical and histological response. The use of imiquimod cream 5% in the treatment of basal cell carcinoma resulted in complete remission of tumors in this trial.
